The sudden death of an apparently healthy infant that remains unexplained after all known possible causes has been ruled out through autopsy, death scene investigation and review of the medical history. SIDS is the leading cause of death in infants between 2 week and 6months of age, with the highest frequency at 3 -4 months, with an approximate rate of 2 per 1000 live births The circumstances surrounding SIDS have been explored in great detail and it is generally accepted that it is a multifactorial condition with variable contributing factors, among the potential environmental factors are a prone sleeping position and thermal stress (overwrapping). Population based studies suggested that heavy rapping, low maternity age and social status and overnight heating may be risk factors for SIDS. The apparently healthy infant suddenly turns blue, stops breathing and becomes limp without emitting a cry of struggling. Most have minor manifestations of an upper respiratory infection preceding the fatal event. Typically postmortal findings are: foamy fluid around the nasal orifices, laryngitis, and fibroid necrosis of chorda tendinae, hyperinflated lungs and petechia. It has also been established that some of the babies have a different bacterial assembly and mitochondrial problems. Pathogenesis The cause of SIDS remains unknown. It has been estimated that about 10% of SIDS cases are due to an inborn error in metabolism. There is an currently agreement that two issues; one is that some of the events thought to occur in SIDS, such as apnoea and abnormal temperature control, are related to neonatal risk factors, the other is the underlying pathophysiology may originate in utero.
